The most important action in solving the problem is formulating the problem; Without a correct formulation of the problem, it is not possible to provide an optimal solution. In this regard, the monitoring issues are one of the most important issues in the governance system, and the weakness in the formulation of these issues will cause the ineffectiveness of the proposed solutions in the monitored area and will ultimately lead to the reduction of social capital, corruption and ineffectiveness of the governance system. Therefore, the current research has been organized in order to formulate the problem of supervision in the matter of governance. To collect data, in addition to studying 13 documents, 24 in-depth and semi-structured interviews were conducted with experts (including 18 government officials and 6 faculty members). Thematic analysis method was used to analyze the collected qualitative data, which resulted in 97 basic themes, 34 organizing themes and 9 comprehensive themes. Comprehensive themes include: lack of effectiveness of supervisory systems, lack of proper implementation and formulation of laws, lack of monitoring the smooth flow of affairs, lack of accountability and responsibility, lack of transparency, lack of coherence and integrity, lack of meritocracy and merit selection, lack of internal monitoring and self-control, and lack of decision-making. A group was identified that expresses the weak factors in the regulatory bodies and the shortcomings in the governing laws and regulations.
